Kids' Zone Waiver

Kids' Zone Permission Slip

Including all rides and activities designated as being part of the Kids’ Zone at the Spring Festival at St. Paul’s Lutheran Church of Milaca, Minnesota on May 10th 2025.  This form MUST be completed by a parent or guardian who will be present at the festival and able to supervise their child(ren) while in the Kids' Zone.

I am the parent/guardian of the child(ren) stated above.
I voluntarily elect for my child(ren) to participate in taking part in the Kids’ Zone rides and activities.
I fully understand that certain rides are dangerous and could cause injury or death.
I voluntarily permit my child(ren) to use the Bounce House ride, knowing of the potential dangers involved.
I assume all risk of injury or death caused by these rides and activities. I will not hold St. Paul’s Lutheran Church, its agents, owners, shareholders, employees, church workers, anyone affiliated with its Management, or anyone involved in any way with the Spring Festival liable for any circumstances of this event or use of the rides or activities thereof.
I hereby confirm that my child(ren) is/are in good physical condition and does/do not suffer from any disabilities or physical conditions that place him/her/them or others at risk or otherwise physically inhibit participation in this event.
By this waiver and release, I acknowledge that I have read, understand, and fully agree to the terms of this waiver and release and its contents. My signature is proof of my intent to execute a complete and unconditional waiver and release of all liabilities in force under the law.
