Application for Assistance with Food or Fuel

Please complete the following form completely, including uploading a copy of your driver’s license or state ID.  Once you submit the application, you will be contacted, first by email, and if no response, then by phone, and if approved, you will schedule a time to come by the church to receive your gift card(s) or other assistance.

Please upload a copy of your Driver's License or a valid State Identification.  We will need:

  • Front, clearly showing your photograph
  • Back, showing barcode or other pertinent information.

If you have a smart phone, simply take a picture of the front, then the back, save them to your cloud (or save to your device and use a USB cable with your device to move to your computer), and upload each image.  Be sure all the text is readable and the photograph is clear.

Benevolence Assistance Rules

St. Paul’s Lutheran Church of Milaca, Minnesota offers limited benevolence assistance to the Milaca community.  Thanks to the generous support of members of St. Paul’s, and the gifts of others in the community, we offer this assistance in accordance with the following guidelines:

  • All applicants must fill out and submit an Application for Benevolence.
  • Priority is given as follows:
    1. Communicant Church Members
    2. Families of Communicant Church Members who reside in the greater Milaca area
    3. Families of Communicant Church Members who are members of an LCMS congregation
    4. Non-members who reside in the greater Milaca area
    5. Non-members who do not reside in the greater Milaca area
  • Proof of need is required at the time of application
  • NO CASH is given at any time except to Communicant Church Members with specific and severe need
  • Benevolence assistance is generally limited to gift cards, gas cards, or vouchers which can be used at local businesses or organizations
  • A valid ID (driver’s license, state ID, etc.) MUST be provided.

While it is our desire to assist everyone as much as we can, due to limited funds we can only offer assistance to those who request it occasionally. 

We provide for the following needs ONLY:

  • Fuel Assistance
  • Grocery Assistance

Those making requests of any other type will be turned away or referred to another local organization better equipped to handle such requests.
